With the flu season approaching, Colonna’s Shipyard will be having an ONSITE Flu Clinic.

Colonna’s will be offering flu shots onsite, Wednesday, October 21, 2020 from 3:00 – 4:30 PM, in the HR Training Room.

You must make an appointment with Yvonne Favors (appointments are 8 minutes)

FREE for Optima Members (bring your insurance card, ID, completed medical release and completed consent form)

$38.00 for Non-Members (bring cash or check, and your completed consent form)

Consent and medical release forms will be sent out separately after scheduling of your appointment. Remember to wear SHORT sleeves!
